DD Services

Community Access Services (CAS)

Community Access Services are designed to assist the participant in acquiring, retraining, or improving self-help, socialization, and adaptive skills required for active community participation and independent functioning outside the participants home or family home.

Community Living Support (CLS) Services

CLS services include training and assistance with activities of daily living (ADLs), such as bathing, dressing toileting, and transferring, with instrumental activities of daily living, such as personal hygiene, shopping, using telephone, and medication and money management.

Community Residential Alternative (CRA)

CRA services include assistance with and/or training in activities of daily living such as bathing, dressing, grooming, other personal hygiene, feeding, toileting, transferring, and other similar tasks.

Specialized Medical Supplies (SMS)

Consist of food supplements, special clothing, diapers, bed wetting protective sheets and other authorized supplies specified in the Individual Service Plan.

Supported Employment (SE)

SE services consist of activities needed to obtain an sustain paid work by participants, including job location, job development, supervision, training, services and supports that assist participants in achieving self-employment through the operation of a business, including helping the participant identify potential business opportunities, assisting in the development of a business plan, identifying the supports that are necessary for the participant to operate a business, and ongoing assistance, counseling and guidance once the business has been launched.

Transportation (Trans)

Services enable participants to gain access to waiver and other community services, activities, resources, and organizations typically utilized by the general population.

New Options Waiver Program (NOW)

The NOW waiver program offers services and supports to individuals to enable them to remain living in their own or family home and participate or live independently in the community.

Comprehensive Supports Waiver Program (COMP)

The COMP waiver program, which serves individuals with more intensive needs, primarily provides residential care for individuals with I/DD. These individuals require comprehensive and intensive services and need out-of-home residential support and supervision or intensive levels of in-home services to remain in the community.

Accessing Services

Who we support

Star Choices Inc. supports people with Developmental disabilities such as; people with cognitive disability, cerebral palsy, autism, traumatic brain injury, and other physical and mental disabilities. We serve adults who experience Intellectual disabilities.

We work with nurses and doctors to assure physical and mental health services are provided to the individuals we support.  Our staff is trained in the specific personal care needs of the person they support and receive additional training on working with individuals who experience intellectual disabilities.

How are people selected?

Star Choices Inc. supports individuals to live in community setting; including; personal home, apartment, host-home setting, etc. . Star Choices Inc. Administration must also ensure the new person’s needs can be met within available resources.

Funding

While Star Choices Inc. would love to support all that need support we are a business and require financial reimbursement for our services. Star Choices Inc. is open to private pay arrangements, in-community support services are very expensive, especially when the person needs 24/7 care or supervision. Most people needing services are referred to Star Choices Inc. through the state Department of Behavioral Health and Developmental Disabilities (DBHDD). Referrals come through the Department’s regional office in Augusta, Cumming, & Tucker. Typically the person has received approval for funding through a Medicaid waiver.
